Description

Functions as a part of an agency team to promote the development and maintenance of client independence and daily living competencies.  Provides direct instruction, guidance and daily living skills support and training to help clients successfully stay or return to their homes. Family Support worker tasks and responsibilities are provided at various times of the day, evening and weekends.  Family Support Workers can be expected to be scheduled outside the agency business hours.
 

Standard working hours for this position will vary, with an emphasis on evenings and rotating weekends.  
Individual schedule will be officially determined after hiring.


Examples of Duties

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive.  Other duties may be required and assigned as best practice or requirements change within the practice field.

  1. Works as part of a multidisciplinary team providing direct service to families.
  2. Assist client’s capacity to perform household tasks, such as: grocery shopping, meal planning and preparation, laundry, clothing selection, acquisition and maintenance, cleaning and other household tasks by teaching, demonstration, active helping and feedback.
  3. Provide support, guidance and feedback to clients. Arrange and/or provide transportation to clients, as appropriate.  This may involve transportation of clients in the employee’s personal vehicle.  Agency vehicle usage is expected unless an agency vehicle is unavailable.
  4. Provide support, assistance, and instruction, as necessary to allow families to manage household finances.
  5. Support and assist families in their interactions with the agency, schools, law enforcement, courts, foster parents, family members and other agencies, organizations or institutions.
  6. Assist families to ensure proper treatment of health conditions by education, support, referral to appropriate resources and monitoring.
  7. Maintain agency records as required.
  8. Perform services as directed by the team leader, assigned social worker or supervisor.
  9. Provide continuous timely feedback to the team.
  10. Assist the team leader in the development, monitoring, and updating of the family’s/consumer’s case plan.
  11. Provide monitoring of clients as directed.
  12. Depending on the designated unit other specific services will be required.  See additional items specific to units below.
     


The following are additional essential functions specific to family support workers in the Initial Assessment and Ongoing Service Teams:

  1. Provides safety monitoring through scheduled and unannounced home visits.
  2. Works with parents on parenting issues and other concerns by acting as a mentor and role model.
  3. Provide input at team meetings based upon observations.
  4. Assist the team leader in the development, monitoring, and updating of the family’s case plan.
  5. Supervise family interaction of children by their parents, and others, as arranged by the agency.
  6. Provide training and guidance in the development of independent living skills to juveniles.
  7. Conduct drug screens.
  8. Assist with engagement of non-custodial parents and family find efforts and connections.
  9. Specific Family Support Worker positions will require a varied schedule with an emphasis on evening and weekend hours.

Typical Qualifications

Training, Experience and Other Requirements: 

  • Graduation from high school or equivalent.
  • Willingness to transport agency clients, utilizing personal vehicle if necessary.
  • Must possess a valid driver’s license
  • Unlimited access to a personal vehicle.
  • A minimum of $250,000/$500,000 auto liability insurance coverage verified upon hire and at renewal by providing a copy of the declaration page of your policy from your insurance carrier.
