The Special Living Services (SLS) program at The Arc Fond du Lac is a community based residential program for people with intellectual and developmental disabilities (IDD) age 18 and older. SLS supports people with IDD in living as independent as possible in their community with access to 24 hour care and services.

Under the general supervision of the Special Living Services Manager, the SLS Case Manager is responsible for managing aspects of four (4) of The Arc Fond du Lac’s Adult Family Home and Community Based Residential Facility. SLS Case Managers are responsible for the care and supervision of intellectually and developmentally disabled (IDD) adults living in a community based residential setting. SLS Case Managers will assist in maintaining the homes’ compliance with regulations set forth by the Department of Health Services; to ensure licensing continuation and exceptional client care. SLS Case Managers main priorities are: menu planning -creating a standard menu for all four (4) homes, responsible for assisting SLS Manager with client intake and discharge, hands on training for new employees, maintaining client files per DHS standards, developing person-centered ISP’s and meeting with families bi-annually, scheduling resident’s medical appointments, proving appropriate documentation, and upholding reporting requirements, incident reporting, and assisting with misconduct investigations. Other duties required as assigned by the Special Living Services Manager or Director of Programs.

The employee is responsible for carrying out The Arc Fond du Lac’s Mission: A human service organization that empowers adults and youth with intellectual and developmental disabilities to become independent and integrated into the community through advocacy, education, training and support.

Special Living Services is a 24 hour 7 days per week operation. SLS Assistants are required to work short shifts in two homes, twice per week. Hours may vary. All SLS employees are required to work some nights and weekends. Flex schedules are an option to provide a healthy work/life balance.

This is a full-time, benefits-eligible position (32+ hours per week) with primary assignment in the Special Living Services Program. Benefits packages available include Paid Time Off (PTO); paid holidays; health, vision, and dental insurance; 401k; and life insurance.


Required Knowledge, Skills, and Abilities

  1. High School Graduate. At least 18 years of age.
  2. Current WI driver’s license and acceptable driving record.
  3. Working knowledge of computers, computer programs, typing, and data entry.
  4. Ability to utilize Google Drive and Microsoft programs.
  5. Dependable, detailed oriented, people oriented, and team oriented.
  6. Complete a background check and pre-employment drug screening.

Preferred Knowledge Skills, and Abilities

  1. Bachelor’s degree in Human Services, Social Work or related field.
  2. Two years’ experience working with persons with disabilities.
  3. Experience with managing crisis situations.
  4. One years’ experience with staff supervision.
