Quad Med is seeking a Wellness Coach to join our Wellness Team! Under the direction of the Wellness Supervisor, the Wellness Coach provides individual and group telephonic and in-person wellness coaching to patients in an effort to cultivate self-efficacy, so that patients can achieve long-lasting healthy lifestyle behavior changes. The Wellness Coach is also responsible for the coordination, and delivery of health promotion activities throughout their geographical area of responsibility to facilitate behavior change, and maximize engagement, outcomes, and customer satisfaction.

Schedule: 24 hours/week; Tuesday, Wednesday and Thursday, with some flexibility

KEY RESPONSIBILITIES

  • Provides appropriate health outreach and wellness coaching via face-to-face, telephone or telehealth technology to reduce or eliminate high-risk behaviors; encourages active adoption of habits that are conducive to a higher quality of life.
  • Assists coaching patients by answering questions, providing program information and education about program services, guidelines and policies in an accurate and courteous manner.
  • Utilizes coaching methodologies and communication skills to solicit change and/or increase a patient’s desire to change behavior and adopt a healthier lifestyle; documents client activities and coaching session notes in established clinical format using EMR technology.
  • Provide services and programs specific to QuadMed’s strategic goals, employee health risks and interests, and collaborates with internal and external resources to provide a comprehensive plan of care with appropriately defined interventions. 
  • Delivers programs including health awareness, educational campaigns, behavior change programs, health assessments/screenings and more, and may assist in the planning and promotion of these programs.
  • Serve as the key resource for engaging employee awareness and participation in Wellness and Health Improvement programs.
  • Serves as a part of an onsite core team to fulfill onsite biometric screening and results consultation stations
  • Provides telephonic/virtual biometric follow-up results consultation/coaching for clients with no dedicated staff 
  • Oversees and ensures all biometric data is appropriately captured and data entered timely into QuadMed reporting system(s) during and after biometric events

JOB REQUIREMENTS

Education

  • Bachelor’s Degree from an accredited college or university in health education, health promotion, nursing, nutrition, exercise science or another related field is required

Experience

  • Minimum one year of experience as a wellness coach required
  • Minimum one year of experience in the delivery and coordination of corporate based health promotion programs required

Certificates, Licenses, Registrations:  

  • Maintain CPR/AED certification
  • CHES (Certified Health Education Specialist) certification preferred 
  • Certification from an accredited wellness coach training program preferred, Wellcoach© certification preferred

Knowledge, Skills & Abilities:  

  • Strong interpersonal communication and customer service skills with the ability to motivate others
  • The ability to work both independently and as a part of a team with enthusiasm, initiative and creativity
  • Ability to make recommendations to effectively solve problems, using judgment consistent with standards, practices, policies, and procedures. 
  • Proven ability to work successfully with diverse populations and demonstrated commitment to promote and enhance diversity and inclusion 
  • General health risk knowledge including cholesterol, blood pressure, BMI and blood sugar required 
  • Strong computer skills required
  • Some travel required
  • Bilingual a plus
