Terms/Schedule – 14 hours per week, 41 weeks/year

Description

The Fond du Lac School District is looking for a Y-Screen Case Manager at .4 FTE. The YScreen Clinician & Case Manager is responsible for assisting with the emotional health screening of adolescents at local high schools, completing clinical interviews, and ensuring that all students who demonstrate a need for help beyond the scope of the program are provided a referral for further evaluation and / or treatment. The Clinician & Case Manager contacts parents to discuss the recommendations, provides information about area resources, and provides ongoing support. Position Responsibilities: Screening adolescents at partner sites and performing clinical interviews with youth who report an elevated level of emotional difficulties or distress. In consult with YScreen Clinician & Site Coordinator, determine recommendations for further evaluation or ongoing services. Telephone the parents of identified students to discuss screening results and treatment recommendations; complete follow-up phone calls as needed. Provide families with information about available community resources (e.g., inpatient and outpatient services, types of therapy offered, payment methods, multi-lingual services, etc.). Develop and maintain working relationships with community health, welfare, and social agencies and seek creative means to assist the youth/family with their needs. Follow case management protocols and complete all required documentation. Input and organization of case-management data. Complete other duties as needed to support the program. Qualifications Master’s in Social Work, Counseling, Psychology, Human Services, or related degree. Applicants with experience in the mental health field preferred. Case-management experience Experiencing working with teenagers and parents/guardians Working knowledge of the DSM-IV Excellent oral and written communication skills Ability to work independently.
