Description

Under the direction of the Release Advance Planning (RAP) Coordinator performs assigned clerical duties and coordination of RAP services to inmates of the Fond du Lac County Jail.Examples of Duties

Duties/Responsibilities:

  • Reviews new bookings daily with RAP Coordinator to assess need for services. 
  • Schedules inmate medical appointments.
  • Communicates with prospective program clients and jail staff.
  • Assist clients with completing program applications and forms. Review for completeness and accuracy. Assist in maintaining program files, client paperwork, medical records, and program tracking sheets/records.
  • Coordinates appointments with Fond du Lac County Jail and other community agencies. 
  • Processes any medical orders.
  • Assists in maintaining inmate’s medical record for MAT Program.
  • Conducts chart audits.
  • Maintains health supplies inventory.
  • Processes orders for program supplies.
  • Assists in maintaining grant budgets as applicable.
  • Completes or assists with reporting for MAT Grant. 
  • Maintains program tracking sheets and compiles program statistics.
  • Complete tasks as assigned by RAP Coordinator and/or community partners.
  • Attends in-service education as assigned.
  • Schedules and attends meetings with community partners and jail staff. 
  • Records meeting minutes and emails minutes/notes to meeting participants. 
  • Assist with Release Advance Planning and/or Vivitrol Program as needed or requested.
  • Provides narcan education and distribution as applicable.
  • Monitors monthly program expenses in order to prepare and submit grant reporting. 

Typical Qualifications

Desirable Knowledge and Abilities:
Ability to follow directions as assigned.  Knowledge of medical office practices, medical terminology, and basic computer applications.  Ability to cooperate with others and work as a team member.  Ability to communicate effectively orally and in writing.
 
Training and Experience:
Graduation from high school.
Must possess a valid Wisconsin driver’s license.
