The Special Living Services (SLS) program at The Arc Fond du Lac is a community-based residential program for people with intellectual and developmental disabilities (IDD) age 18 and older. SLS supports people with IDD in living as independently as possible in their community with access to 24-hour care and services.
The SLS Case Manager is responsible for managing aspects of The Arc Fond du Lac’s Adult Family Homes. SLS Case Managers are responsible for the care and supervision of intellectually and developmentally disabled (IDD) adults living in a community-based residential setting. SLS Case Managers will assist in maintaining the homes’ compliance with regulations set forth by the Department of Health Services; to ensure licensing continuation and exceptional client care. Special Living Services is a 24-hour 7 days per week operation. SLS Case Managers are required to work short shifts in two homes. Hours may vary. All SLS employees are required to work some nights and weekends.
Qualifications
Required Knowledge, Skills, and Abilities
- High School Graduate. At least 18 years of age.
- Current WI driver’s license and acceptable driving record.
- Working knowledge of computers, computer programs, typing, and data entry.
- Ability to utilize Google Drive and Microsoft programs.
- Dependable, detail-oriented, people-oriented, and team-oriented.
- Complete a background check and pre-employment drug screening.
Preferred Knowledge Skills, and Abilities
- Bachelor’s degree in Human Services, Social Work, or related field.
- Two years experience working with persons with disabilities.
- Experience with managing crisis situations.
- One year’s experience with staff supervision.
